For Summer Sessions: ALL athletes and Marching Band/Color Guard participants MUST turn in both the Health History Update Questionnaire
and the Summer Permission Form prior to participating. Both forms can be found below.
**If you are a new student to the district or participating in a sport for the first time, please contact your school nurse via email (see emails below) prior to registering in order to have access to the registration process**
Students registering for Athletics must have a current physical examination on file in the nurse’s office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in prior to the dates mentioned below.
Physicals are valid for 1 year from date of the last exam. Download your NEW Sports Physical Form HERE.
If the athlete has a current physical on file in the nurse's office , you DO NOT need a new physical. Please submit the Health History Update Form instead. An original copy must be submitted to your school nurse. Download the form HERE.
Prior to attending EACH summer session, every student-athlete MUST complete and bring with them the COVID-19 Daily Pre-Screening Form. Without this form, you will NOT be permitted to practice that day. Download form HERE.
If the athlete has Asthma and uses an inhaler please click HERE.
If the athlete has an allergy that requires the use of an Epipen please click HERE.
Attention High School Athlete's ONLY: If you are going to be participating in any OUT OF SEASON workouts, you MUST complete the corresponding season form and return to Mrs. Redler in the nurse's office prior to participating. Otherwise, you will not be permitted to participate. Click on the word "HERE" next to the appropriate season for the form you need.

Please Note: Final clearance will NOT be granted until both the online registration process and original Physical/Health History Update forms are completed and have been received by your school nurse and our school physician signs off.
