****Physical Examinations - IMPORTANT UPDATE BELOW:****
NEW ATHLETIC FORMS NOW IN USE - Starting July 1, 2024, old physical packets will NO LONGER be accepted. Please DO NOT submit the old physical packet after July 1, 2024. They will not be accepted and you will be required to have your physician complete the new ones. This will increase the time needed for final clearance. All forms can be found below and on your school’s athletic page under permission forms.
Attention Incoming 9th graders - if you played a sport/intramurals/marching band at the Middle School and already have an account, you do NOT need to create a new one. Just click on top icon - Family Account - Return User Login. All accounts transfer over from the middle school to the high school.
**If you are a new student to the district or participating in a sport for the first time, please contact your school nurse via email (see emails below) prior to registering in order to have access to the registration process**

MUST BE ORIGINALS.
Please turn all forms in to the main office or the nurse's office.
Physical Examinations
Students registering for Athletics must have a current medical eligibility form on file in the nurse’s office before the student is authorized to participate in practice/tryouts. To ensure approval for participation, physicals should be completed and the medical eligibilty form turned in prior to the dates mentioned below.
Physicals are valid for 1 year from date of the last exam. Download your NEW Sports Physical Form HERE. Please note - only the Medical Eligibility Form of this packet MUST be returned to the school nurse upon completion of the physical. All other paperwork should be kept at your students physician's office.
If the athlete has a current physical on file in the nurse's office , you DO NOT need a new physical. Please submit the Health History Update Questionnaire instead. An original copy must be submitted to your school nurse. Download the form HERE.
If the athlete has Asthma and uses an inhaler please click HERE.
If the athlete has an allergy that requires the use of an Epipen please click HERE.
Attention High School Athlete's ONLY: If you are going to be participating in any OUT OF SEASON workouts, you MUST complete the corresponding season form and return to Mrs. Redler in the nurse's office prior to participating. Otherwise, you will not be permitted to participate. Click on the word "HERE" next to the appropriate season for the form you need.

Please Note: Final approval will NOT be granted until both the online registration process and original Medical Eligibility Form or Health History Update form is completed and have been received by your school nurse and our school physician signs off.
Please be mindful if paperwork is late, that we can not guarantee approval for participation on the start dates.
**Please note that NO student will be allowed to participate in official practice until completed forms are submitted and reviewed by our school nurse and school physician for approval. Therefore, if forms are submitted after the due dates, there may be a delay in approval.
